How to purchase DIVA’s for more than one clinician:

When purchasing the DIVA for example, 5 users, mark that you are buying 5 items. Be aware that you will receive 1 invoice with a single code for all 5 users. Distribute this code to the other users.
Another way of obtaining 5 DIVA’s for 5 users is that each user should purchase/download the document herself/himself. In other words, 5 documents will have to be purchased separately: 5 invoices with 5 different codes, 5 documents to save and 5 order confirmations with the download link.

There is also a possibility for two e-mail addresses to be entered into the system:

Several email addresses can be entered into the system, for example of the sales department and of the customer. When two email addresses are entered, the invoice and order confirmation will go directly to the sales department. The customer also needs to see the invoice and the order confirmation as it contains the code and the link to download the document.

Be aware that you must download the document within two weeks otherwise the code becomes invalid.

You need to contact us to tell us you wish to build the DIVA-5 into your online platform. Once this is done, we will check your algorithm. The costs are € 650 Euro. You then pay us € 4,95 Euro per download once it is in your platform and approved by us. We will send you a contract if you choose this option. To issue the invoice, we need a name and address, if applicable a purchase-order and a VAT-number, so please supply these in your email to ue.retnecavidobfsctd-68aca3@ofni.

Yes, you can! You can use the DIVA-5 for your staff in the following way in a clinic setting:

1. You inform us how many users you have in your clinic who will be administering the DIVA. You also inform us which DIVA version you want, remember we have 3 versions of the DIVA: Young DIVA-5 for children and adolescents, DIVA-5 for adults, and DIVA-5 ID for those with intellectual disability.
2. We then send you an invoice for the number of users (n). The costs are € 10,- Euro per clinician per DIVA. You purchase one PDF from our website (costing € 10,- Euro). You have two weeks’ time to download the document. Use the document as a template. You use the code that is generated in your direct online purchase for each of your clinicians who will be using it. The clinician can use the DIVA pdf an unlimited number of times. If you have 10 clinicians, the total costs paid to us are € 100,- Euro for which we will send you an invoice.
3. We work on an honesty system, so these codes should not be shared with people outside of the users in your clinic. If more users join your clinic, you let us know and we invoice you for the additional number of users.

1. Yes, you can! But please note, the DIVA needs to be translated from Dutch into your language. There is usually an entry fee of € 2000.- Euro per language, but we are now waiving this fee. We can make a rough translation for you from Dutch into your language using AI. We will send this to you, and you will need to check the words used in the translation are the correct ones used in the treatment and diagnosis of ADHD.
2. If you yourself cannot do this, you will need to find psychiatrist(s) or psychologist(s) who is/are fluent in your language and who can check the words used in the description of the DSM-5 criteria for accuracy. The vocabulary used in the DIVA should match what is currently used by mental health professionals in your language. All colleagues involved in the translation must be added to the Colophon.
3. Once you have checked and corrected the document we send you, you need to email it back to us as an UNFORMATTED Word document.
4. We then use a separate translator to translate your language back into Dutch. We will check the accuracy of the back translation.
5. The references do NOT need to be translated.
6. Please make sure the reference numbers are in the correct place in the introduction text.
7. Please remember that we need a Word document that is UNFORMATTED. In the end, we will create the PDF.
8. We will of course send our PDF back to you to be checked and proofread. We will also send the informed consent document. Please contact us at ue.retnecavidobfsctd-22bbf9@ofni so we can move ahead with the new translation.
9. Once the PDF is published online, it will be your task to spread the word in your country.

At least 3 symptoms need to have been present before the age of 12 years. For adolescents aged between 12 and 17 years, at least 6/9 symptoms need to be present of either Inattention (6 in total), or Hyperactivity/Impulsivity (6 in total) or both Inattention and Hyperactivity/Impulsivity (this would be 12/18 in total). These may be the same or other symptoms as before age 12.

Over the age of 17, at least 3 symptoms of either Inattention, or Hyperactivity/Impulsivity need to have been present before the age of 12. PLUS, currently, there need to be at least 5/9 symptoms of either Inattention (5 in total), or Hyperactivity/Impulsivity (5 in total) or both (10/18 in total) to diagnose this neurodevelopmental condition.

Yes, if the person does not recognise the symptom the clinician may go through the examples listed. In practice, the clinician can read (some of ) the specific examples listed under each symptom to determine whether the symptom is present or not.
